Summary
This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties
- The Advanced Practice Nurse - Endocrinology/Diabetic Educator is a Nurse Practitioner (NP) who provides comprehensive - patient-centered care for Veterans with diabetes and related endocrine disorders
- The incumbent functions with a high degree of clinical autonomy and is responsible for assessment - diagnosis - treatment - and management of complex diabetes cases
- The NP integrates clinical expertise with patient education - population health strategies - and interdisciplinary collaboration to improve glycemic control - reduce complications - and enhance quality of life
- The role includes direct patient care - diabetes self-management education - staff training - and program development consistent with VHA mission and performance measures
- Duties and Responsibilities include but are not limited to the following: Performs comprehensive history and physical examinations with emphasis on endocrine and metabolic conditions
- Diagnoses and manages acute and chronic diabetes conditions - including Type 1 - Type 2 - and complex insulin-dependent cases
- Develops and implements individualized treatment plans - including pharmacologic and non-pharmacologic interventions
- Prescribes and adjusts medications - including insulin regimens - GLP-1 receptor agonists - and other antihyperglycemic agents in accordance with VA privileging
- Orders and interprets diagnostic and laboratory studies relevant to endocrine disorders
- Manages diabetes-related complications - including neuropathy - nephropathy - and cardiovascular risk factors
- Provides care coordination across inpatient - outpatient - and specialty services
- Provides structured diabetes self-management education (DSME) to Veterans and caregivers
- Educates patients on glucose monitoring - insulin administration - nutrition - exercise - and lifestyle modification
- Promotes adherence to treatment plans and facilitates behavior change using evidence-based strategies
- Assesses health literacy and adapts education to meet individual Veteran needs
- Supports use of diabetes technologies - including continuous glucose monitors (CGMs) and insulin pumps where applicable
- Participates in population health initiatives targeting improved glycemic control and reduction of diabetes-related complications
- Utilizes data dashboards and registries to identify high-risk patients and implement proactive interventions
- Supports performance improvement initiatives aligned with VHA quality metrics (e.g. - HbA1c control - complication prevention)
- Assists in development - implementation - and evaluation of diabetes management programs within the Endocrinology Service
- Collaborates with endocrinologists - primary care providers - pharmacists - dietitians - and nursing staff to optimize patient outcomes
- Provides consultation to other services regarding diabetes management
- Participates in case conferences - care planning meetings - and interdisciplinary rounds
- Serves as a clinical resource and educator for nursing staff - providers - and trainees regarding diabetes care
- Develops and delivers educational programs on diabetes management and endocrine disorders
- Precepts nursing students - nurse practitioner students - and other trainees as appropriate
- Participates in quality assurance and performance improvement activities
- Ensures compliance with VHA policies - clinical practice guidelines - and accreditation standards
- Contributes to development of clinical protocols and standard operating procedures
- Maintains accurate and timely documentation in the electronic health record in accordance with VHA policy
- Incorporates evidence-based practice into clinical care and education
- Participates in research and program evaluation activities as appropriate
- Supports implementation of best practices in diabetes and endocrine care
- Preferred Experience: Specialized experience in diabetes management and endocrinology
- Certification as a Diabetes Care and Education Specialist (CDCES)
- Experience with insulin pumps - continuous glucose monitors (CGMs) - and advanced diabetes therapies
